Long Term Conditions (LTC)

The LTC service is provided in all community pharmacies to support patients with identified medication adherence issues to become self-managing with their medications.

Canterbury has consistently had lower than national levels of LTC registration. Pharmacists are encouraged to continue to be proactive in registering LTC patients who are eligible for this service and may benefit from it.

Who qualifies for the LTC service?

  • People who have difficulty managing their medications and/or illnesses who would benefit from adherence support and monitoring.
  • These people must also have at least one long term condition such as diabetes, heart or respiratory problems, depression, hypothyroidism or arthritis.
  • They must live independently in the community and not live in an aged residential care facility or community residential care facility.
  • They must reach a 20 point threshold in the LTC assessment.
Long Term Conditions Pamphlet
This pamphlet explains the LTC service to potential LTC patients and is in large print for ease of use. It incorporates a consent form which has been approved by the DHBSS with the proviso that:
  • The patient is provided with the health information privacy statement
  • The patient ticks the box on the consent form which they then sign against to confirm they have been provided with the health information privacy statement.
  • The patient understands what they are agreeing to by being registered in the service.

    This pamphlet from DHBSS includes a checklist for patients to self-identify that they have adherence issues. It leaves it open for the pharmacist to decide with the patient what service is appropriate.

    LTC Auditing
    • The Community Pharmacy Service Programme will be making a LTC self-assessment audit tool available in 2016.
    • The aim with the new audit tool is to set a minimum standard as is necessary for contractual obligation. Then also to encourage a quality improvement approach with recognition for pharmacies providing more than the bare minimum.
    • Currently audits are carried out retrospectively, looking at LTC registrations and activity over the previous 12 months.
    • Registration forms will be looked at, as well as a random sample of 20 patients in each pharmacy being looked at in depth.
